How do I find the office?
Our address is 336 South 10th Street, Suite 107, Montrose, CO 81401. Our building can be a little confusing, here is the door you should be looking for:

If you would like us to obtain your past medical records, please complete this form and send it in to our office. We will work with your previous physician’s office to obtain your records.

If you would like us to release your current medical records, please complete this form and send it in to our office. We will get your records sent to the office of your choice.
